In order to obtain copies of your medical records or x-rays please complete the "Records Release Form". After you have filled it out, please return it to our office either in person, by mail, or fax (732-998-8086). We will only release records generated from our office. Medical records from other providers will need to be obtained directly from that provider.

Getting ready for your appointment with Bayside Orthopedics

To save time, please print out the "New Patient Registration" forms and fill them out at home to bring with you to your first appointment.

The New Patient Registration may be completed online and printed if using the following internet browsers:  Chrome or Microsoft Edge.  If using Firefox the form can only be downloaded for printing.

If you are a new patient, please arrive 20 minutes prior to your scheduled time to complete our registration process. In addition, please bring the following:

Please Complete Our Patient Forms and Print Out the Patient Forms, if possible
Picture ID: Driver's License or State ID
Insurance Information (cards, if applicable)
Copies of X-rays, MRIs, CT Scans, & Operation Records from Other Providers
Copy of Written Referral, if applicable 
List of Current Medications and Any Known Allergies
